At ccc, Our Post Hospital Placement, Accommodation and Support program will work with you and your family/carers to move you into temporary and safe accommodation with the necessary support required to help you until you can safely move back home.
We have many specialised and highly trained staff who have the right skills and abilities to meet the particular needs of individuals with complex care needs.
This helps gets you out of hospital while we work hard to speed up getting your home modified or getting you assessed for Specialist Disability Accommodation (SDA) funding from the NDIS so that, if approved for SDA funding, you can move into a new purpose-built home.
